Most of this was either work-related, part of routine family trips, or happened during a cross-country trip in 1987. My standard was "visited an attraction or spent the night"; this eliminated many of the states we merely drove through during that cross-country drive.
I didn't include the map of the world, because there's only a handful of other places (so far):
* Canada (came back the long way from Michigan one year, and again with work while in Detroit)
* France (1985, school trip)
* Japan (2002 on my own; 2005, 2006, 2007, and 2008 for Otakon)
* Dominican Republic (2008, with work, but on vacation)
Next year, the plan is to hit the UK.
