I've seen it happen several times, and have been on the receiving end a few times. Someone requests to be your friend. Weeks later, you try to follow up with an e-mail, which they promptly ignore. Okay, so what the hell is the point of adding me, other than to make your pathetic friends list look good?
Conversely, don't accept my friend request either, if you have no intention of talking to me.
I'm faced with a similar dilemma on my journal. I recently received a request to add someone as a friend into my journal. This was back on 10/9, and I figured I'd go ahead and add her. She hardly posts and has yet to return the favor on her end. I was almost tempted to de-friend her, but I figured I'd give it some more time. Well 30 days have nearly lapsed and I have not seen any activity from her. If she doesn't friend me, or at least make a comment in my jornal in the next week, she's going to be on the chopping block.
