Different publishers do it differently, and do it differently for each book. For example, the Order of the Air covers are original paintings commissioned from Bob Eggleton. They're absolutely unique and lovely! The General's Mistress cover from Simon & Schuster is a photoshop of two 19th century paintings. I had two sets of covers for Black Ships and Hand of Isis, the first ones digital manips from their artist in New York, and the second ones are original paintings by John Jude Palencar.

I've always designed my own novel covers, since I do photography as a hobby, but I'd be curious to know how it's done for regular novels also.
