No, you're not being obtuse I fear I was being too vague... but I wasn't sure how to deal with the topic.
Let's say that Subaru, after what Seishiro did had been under strong depression, trauma and pressure. To men this can be a very bad combination that might cause temporal impotence. The fact that his clan took the problem at heart (they wanted heir and Subaru couldn't and wouldn't be cooperative) only worsened things. Subaru took it in his usual optimistical way, which meant he believed it to be a permanent problem. -_-
Being out by the hospital by not much time after having been nearly killed as he wanted and being denied his wish plus being 'jumped over' didn't help either.
Really, when Seishiro has the feeling the world might be about to die and he might not have another chance with Subaru he should start from the romantic dinner and then move slowly... -_-
