Closer and closer!

So now that I have my application printed out...where do I sign it? That and the self-assessment Medical Report. Do I just sign in the empty space below the last question? There's not a place to sign it :O

Also, do I need to print out the self-assessment medical report separately? It's already contained within the application itself.
