We've got a knack for fucked up history

Why do we write 'Dear' on letters to people we don't know? It's always struck me as odd, I've always tried to avoid it, in formal correspondence in particular.

Where does this false intimacy come from? We don't use it in conversation, or over the phone, but it's ingrained into us from childhood, and exists in the formal templates for letters.
