Use XVID AVI files in SD or 720 instead of
MKV/H264/X264
Use Nlited Windows 2000 or 2003 instead of
default XP or Nlited XP or bloated vista/vlited vista
Consider using Linux HTPC distro instead of windows
Defrag all hard drives with perfect disk
Use updated stable hardware drivers not old drivers off cd/built into Windows.
Make sure PATA optical drive is not connected
to the same channel as hdd .
Make sure Hardware acceleration is turned on
Update DirectX9c to the latest build
Use Quicktime Alt Codec v177 (supports MP4-AVC/H264)
or pay commerical COREAVC thingy codec
Use a video card that can help play back of H264/X264
like Nvidia 7600 and above cards

P4 cannot handle MKV 720 that well. I can give you one suggestion try install CoreAVC codec from

CoreAVC codec uses less CPU on MKV 720 decoding than VLC, so give it a try.
go and configure ffdshow video and disable h264 decoding with ffdshow.
right click -> filter -> ffdshow video decoder -> codecs (on the left column) -> (on the right hand side) click H.264/AVC from libavcodec to disable. close mpc then start the video again.
after that, right click again and find coreavc video decoder, then set deinterlacing to hardware.
t might be because its using ffdshow audio decoder then passing it to something else. for my one, it goes through ffdshow audio decoder then ac3filter. (my) ffdshow mixer changes any multichannel audioed video to 2.1 output (by default) and passes this along to the ac3filter before it goes to the speakers. so it doesn't work properly.
for ffdshow you probably need to set the mixer to use its input as its output directly. then let whatever else handle the audio. for me, with ac3filter, i would then need to set the output format as 3/2+SW 5.1 channel. this way audio is still 5.1 at the output of the ac3filter. you should do a google search and download and install the latest ac3filter.
if you right click you should be able to see the other audio decoders in there being used to play that video file. but not all of the audio decoders will be named as a audio decoder (like ac3filter). once you go into ac3filter config panel, you will be able to see its input and output formats. if the input levels (on the right) isn't 5 channeled then theres probably is something wrong higher up in the audio decoding chain.
