The estructure would be showing parallels situations in three different settings. It would contrast morning rituals, the landscape of every city, the infrastructure of each hospital, and identify characters based on what our three leads call them more than their actual names. Three leads with very different specialities, in very different hospitals, going through the same things (becoming surgery attendings), dealing with more or less the same kind of people and problems (love drama, cases, work conflicts) and having a similar outlook in life even though each of them has a different attitude about it. The night comes, and they chat on the phone or facebook or skype, snark a lot, gloat even more, and exchange experiences.
In the long run it wouldn't work, but because we are already invested in the characters, I would totally follow this story around!

Joss Whedon as Grey's' showrunner... ummm, that's a very different show, and at the same time... I like the way Whedon works season arks and how he pays attention to continuity (when it's his own). Then again, Lexie would have died anyway. Alex would have died in the shooting, for sure! And if not, Izzie would have died instead of leave (which sometimes I kind of wish it happeneded). Meredith would be just as dark and twisty and kick ass in what she does, I think. Same with Cristina. I guess the execution of the show would be a lot better, we wouldn't have so many useless or repetitive plots, which is my main problem with the show, but the characters would be pretty similar, I think. Same bad habits, same mistakes, same interaction, more or less... it's interesting to realize the similarities between character constructions...
