The grey countries are the ones without comprehensive public health care systems... you know, like China, the Sudan, Mali, Ecuador, Kazakhstan, the United States of America.
Note, however, that there is universal public health care, at least to some extent, in Afghanistan and Iraq... and guess who's paying for it?
