Ah, the opening scene is describing Hae's first rebirth, starting with him lingering in I guess what would be purgatory, the margin between life and death. Coming more into himself he has a consciousness filter that allows him to register outside stimuli (such as Geng and Siwon talking about him and nursing him back into life), but confuses it with his memories of Hyuk (and, perversely, swallowing his come during hebia). Memories in the form of taste filled his mouth: warm and seedy, coating his tongue with life and love, pulsing on the back of his throat. Fingers on his jaw, kind and loving, a whispered Good baby, swallow it all.
Donghae is actually reborn twice in this chapter. There's the first one, where he is only brought back half-developed (hence his weakened, malnourished state). When Siwon rapes him, he kills him again, but does this so that Hae can come back as a superior Ipsuren, not really a God figure as Siwon is, but I guess a demi-god? He's Nokhiri, but he definitely has half of Siwon in him now. So that was the point of that. Even though it was horrible and brutal and sadistic and terrible, it was a necessary evil. All for the sake of plot progression!
