They get a second surprise that day after previously finding out that the man, no older than Dean or even younger, turned out to be their grandfather, their dad's father, Henry Winchester.
Or rather, a surprise for Dean. Or two.
It's Dean's birthday today so Sam has slipped earlier to pick a round pan of pecan pie from the diner across the street. He placed an order the day before just to make sure they wouldn’t run out of it again. When he and Dean talked the other day, Dean told him about how he finally got to a place where they had pecan pie only to find out not five seconds later that there was no more pie.
Eyeing the brown-bagged box in his hand, Sam smiles to himself rather proudly. Dean can have as many slices as he wants this time.
“Morning,” Sam says, pushing the door open.
“What’s with the stars and sunshine so early in the morning?” Dean pours himself a coffee and lifts the jug to Henry. “Coffee?”
“Sure, thanks.”
“Stars and sunshine? Dean, it’s your birthday today.” Sam pushes the box to under Dean’s nose. “I got you this.”
“Really?” Dean’s smile cracks. Sam knows Dean hates talking about birthdays but when it comes to pies, nothing can beat them. “Hmm, it smiles like pie.” Dean’s eyes gleam toward Sam. “It’s pie, isn’t it?”
“So it’s your birthday,” says Henry. “That’s great. Happy Birthday to you, Dean.”
“Yeah, yeah.” Dean waves dismissively. “I have a more important thing at hand at the moment aka this pie. Sam, you’re a genius!”
Henry’s smile gets wider. “I’m glad she decided to keep you. It was hard, we knew, but she had to pick herself up together. After all it was almost a year after-”
“What are you talking about?” Sam gapes, turning to Dean, who shrugs at him.
“… a year after she lost her firstborn,” Henry finishes his sentence, which sends both Sam and Dean into a complete and utter, shocked silence.
“What?” Sam gazes at Dean and switches to Henry. “You mean Dean’s not-”
“Oh Jeez.” Henry looks stricken. “You didn’t know. I thought-I thought your mother had-”
“No,” Sam snaps and jumps in surprise at the hard slam of the door. Dean has disappeared from the room. Sam swirls back to Henry. “What have you done?” he roars and dashes out. So much for the pie.

Sam grinds into a halt as he catches the sight of the Impala sitting quietly in the corner of the parking lot. He takes a deep breath when he spots Dean, too, sitting with hunched shoulders under the billboard showing the name of the motel next to the gate. Sam straightens up and goes to Dean.
“Hey, you okay?” he says with a low voice.
After a short silent moment, Dean stirs. “That explains everything, right, you being dad’s favorite while me, after all those efforts trying to be a good little soldier, dad was still-”
“It was never like that.”
“Come on, Sam.”
“No.” Sam grabs Dean’s shoulders and pulls him up on his feet. “Dad gave his life to you, remember that?”
“Sam, you have no idea…”
“No, I don’t. But you’re a Winchester through and through. You’re my brother. No one can take that away from me. If there’s one good thing that comes out of this-”
“What?”
“The fact that we’re not blood related makes it so much easier for me to do this.” Without waiting for Dean’s response, Sam pulls Dean’s face close to his and kisses him gently on his lips. He caresses Dean’s cheeks with his thumbs as he withdraws slowly. “I’ve been waiting for this all my life but I don’t know how to do it without freaking you out.” He takes in Dean’s dazed face and chuckles a little. “I love you, Dean. You have a problem with that?”
“I, umm.” Dean swallows hard. “I don’t think so, Sammy.”
“Good.” Sam cups Dean’s face again with both hands and kisses the corners of Dean’s mouth first before pressing their lips together even more tenderly this time.
“Happy Birthday, Dean Winchester. Now, shall we have that pie?”
