He forges little changes when he's dreaming, mostly without consciously thinking about it, and in reality only he knows the balance and weight of this particular penis he can't. Plus it means he can adjust himself in public whenever he feels like it for totally legitimate reasons...
Arthur (or Yusuf, or anyone actually) finds out and thinks he's an idiot and is a little turned on anyway. Especially if, once found out, Eames starts deliberately touching himself when he's being watched and is all casual and “what? I'm just checking my totem” if he gets called on it. Or offers to let someone touch his totem as the ultimate sign of trust and not just to get a little action, honest.
Established relationship, or first time, or just Eames being a dick. It's all good :D
