NOTE: Inaba Province exists except it uses different kanji from P4's Inaba. I'm taking creative license based on information from the game.
After the twin creation gods, Izanagi and Izanami, created the islands that made up Japan, a young boy appears on the neighbouring land of China. He is the personification of Japan.
As Japan grew up, learning from China before leaving him, he's heavily influenced by his people forming into groups, building up villages, towns and cities. He then gave 'birth' to various personifications of cities and towns, where the population has formed their sense of community and identity. Only the Emperor and high-ranking government officials knows the personifications' identities, seeking to protect them from other international powers' bad influences.
Inaba, under the alias of Souji Seta, is one of Japan's children and grandchildren of Izanagi and Izanami. He is one of the older personifications among his siblings. As a near-immortal being, he moves around the country every few years to prevent people from being suspicious of his lack of aging. He would always go back to Inaba the city, where he can walk among his people and listen to their problems, helping to relieve some burden from Japan's shoulders.
When the events of P4 happens, he would try to juggle the investigation, his 'normal' human life and his obligations to the local politics without spilling his identity.
Ryotaro Dojima acts as his 'guardian' and 'uncle', in order to keep up the facade, even if Nanako believes them to be related and loves her Big Bro.
Inaba - Souji Seta
One of the older personifications. He used to be an old mining town, before the modern age comes and grown accordingly. However, he's still rather calm and quiet, and a mixed between old-fashioned and modern, due to the adults and older people, and the younger populations.
Over the years, he occasionally hear rumours about something special. Like the Midnight Radio and now, the Midnight Channel.
...Until the Midnight Channel was proven to be real.
Iwatodai - Minato Arisato (iwatodai_tan )
An artificial, man-made island. He was found when Japan and the Kirijo Group's president at the time tour the construction site. The events of P3 happens as normal, except with a few key differences. For example, Iwatodai knows Mitsuru Kirijo when she's young.
As a personification, 'Minato' didn't die from using the Grand Seal, for as long as people continue to live on 'him', the area. However, the Seal restricted his movement outside of his area. Which leads to his friends and brothers to come to visit him.
Tartarus/Dark Iwatodai - Ryoji Mochizuki
Both the personification of Tartarus and Nyx Avatar. He was born as a personification when, as Death, he was sealed inside 'Minato', Iwatodai's personification. After the Grand Seal, he now exists as the other side of Iwatodai, living together with 'Minato'.
Mikage-cho - Naoya Toudou
Sumaru City - Tatsuya Suou
Tokyo - Raidou Kuzunoha (risingsunfox )
Although the personification of Tokyo is Raidou, the large size of the city almost made it necessary for each districts/wards to have their own personifications to help maintain it. Overseeing them all the districts is, of course, Tokyo itself, 'Raidou'.

> Chuo - Naoki Kashima (reverseditall ) - The Conception happened, but 'Naoki' managed to restore the world back to normal (Freedom/Neutral Ending). Got himself a pat on the head from 'Raidou'.
> Minato (Minato-ku) - DeSu MC-tan. The Tokyo Lockdown happened and was the only one in position to deal with the crisis. He solved the problem by choosing to follow Gin's plan: sending the demons back and re-establishing the barrier between the human and demon world.
