Dean's siren-induced spillage is hurtful in its own way - Sam's always suspected Dean felt that way (since way before the current season) and Dean's always been quick to deny it until that moment...
I agree that it comes down to trust - not just that Dean can't trust Sam but also that Sam can't trust Dean either. Dean thinks Sam believes he's weak; Sam thinks Dean believes he's going dark side and changing to the point he's not Sam anymore ("If I didn't know you I'd want to hunt you", anyone?). The lying, like the powers, seem to be a reflex now and that's bad, but I don't blame Sam for not wanting to confide in Dean about this when he thinks Dean believes this.
The "DEMON BAD" isn't gone from Dean's psyche - if Sam was lying about talking to Bobby, Ellen, or pretty much anyone else they've had prolonged encounters with, Dean would be suspicious, yes, but he wouldn't be immediately assuming the worst. Because it was Ruby, however, Dean's mind automatically went to the worst possible scenario...
I was so thrilled when Castiel said he recruited both Dean and Sam - YOU TWO ARE IN THIS TOGETHER BOYS! GET YOUR ACTS TOGETHER!
