The rift in Time that the Cybermen use in Army of Ghosts/Doomsday already existed - Torchwood One just spotted it, and had the Torchwood Tower built there to try to explore/exploit it. Oh right! *slaps forehead* I completely forgot about that. But that makes me wonder how common such rifts are. (Bermuda Triangle?)

I need to rewatch "Turn Left" and try to remember the nature of that problem. It was a parallel timeline created by the Time Beetle (related to the Trickster in Sarah Jane Adventures episodes who also tweaks timelines), right? Did that constitute an alternative universe? What is the nature in New Who of alternative timelines and their relationship to alternative universes? Hmm. Lots of thoughts to chew on.

I've seen refs to either fic or tie-in fiction which mentions the Bermuda Triangle as another rift.

I need to rewatch "Turn Left" and try to remember the nature of that problem. It created an alternate timeline, not an alternate universe. But that wasn't what I was referring to when I mentioned Rose popping back and forth - I was talking about her appearances elsewhere in S4, rather than in Turn Left. Although I can't help thinking that popping back and forth in an alternate timeline is no less dangerous than doing so in the established timeline...

I've been interpreting that scene from the trailer as a flashback to the end of the Time War, not something the Time Lords are currently discussing from their secret underground bunker. But as for how they survived -- the spoilery speculation I've seen is that they're hiding out in the Master's head. No, I don't understand how that works either -- RTD logic is not Earth logic -- but it would explain why the Doctor hasn't been able to sense them. (And potentially also explains your paradox about how they could be simultaneously discussing both the Time War and the Doctor's current situation.)

Yes, I have been interpreting the scene as a flashback to the end of the Time War and you're right that your spoilery speculation could explain away the paradox that I feel is inherent this Part 1.

The more that spoilery speculation sticks in my brain, the more plausible it seems... although I can't point to any one thing that makes it seem plausible.
