Made in Adobe Photoshop 7.0, but it's easy translatable to other programs.
I used this pic.
1. Resize and crop your picture to 100x100
2. Use an unsharp mask with the following settings:
Amount 80%
Radius 2.0 pixels
Threshold 4 levels
3. Take your blur tool at 30% strenght and smooth out his face/skin (avoid his eyes,lips,etc)
4. Duplicate your base twice. Set the middle layer to Screen @ 50% opacity and the top layer to Soft Light @ 100% opacity. Merge layers.
5. Take your Rectangular Marguee tool and select a part of the pic. Copy and paste it. Rotate it 90 CCW and flip it horizontal. Desaturated it. Move your base layer to the left (see pic)
6. Duplicate the desaturated layer and set it to Screen @ 100% opacity. Merge down.
7. Add this brush (

8. Erase the parts from the desaturated layer so it fits in the border. Do the same with the colored pic.
9. Time to add some text and brushes. I wrote 'sexy' in capitals and with the font 'violation' on 14 pt. I sharpend the text. Then I added this brush in a hard pink color (#FC1F5D). Sharpen it.
