I used this picture of Amanda Seyfried.

1) I resized the picture to 70*105.

2) Open new 100*100 page and fill it with this color: c0bcb2#

3) Add this texture and set it to Soft Light.
4) Layer > New Fill Layer > Gradient


Set it to Hard Light, Opacity: 20%

5) Layer > New Fill Layer > Gradient


Set it to Soft Light, Opacity: 40%

6) Now add your base (I mean the picture that you resized)

Your icon should look like that:


7) Layer > New Adjustment Layer > Selective Color

Reds: -96, +13, -25, +57
Yellows: +33, -39, -3, +31
Greens: +36, 0, -61, -81
Whites: +29, -50, -49, -26

8) Layer> New Fill Layer > Solid Color
Choose this color: f2b9b9# and set it to Color Dodge, Opacity: 20%, Fill: 50%

9) Add this texture and set it to Lighten.
I  moved the texture up a little bit and erased the texture on Amanda's face.



10) Layer > New Adjustment Layer > Hue/Saturation
Master: -1, +16, 0

11) Layer> New Fill Layer > Solid Color
 Choose this color: 9aa9e7# and set it to Luminosity, Opacity: 20%

12) Layer > New Adjustment Layer > Curves
RGB

First Point
Input: 146
Output: 184

Second Point
Input: 109
Output: 125

13) Add this texture and set it to Soft Light, Opacity: 25%

14) Layer > New Fill Layer > Gradient


Set it to Overlay, Opacity: 25%

The icon look like that:


15) Merge down all the layers (Ctrl + Shift + E),
duplicate your icon and lower the opacity to 60%
Sharpe that layer ( Filter > Sharpen > Sharpen)
And than do that: Filter > Render > Lighting Effects


Press OK and you're done!
