REASON FOR VISIT: Evaluation for Project Twilight.
HISTORY OF PRESENT ILLNESS: Patient presents for physical evaluation, specific physical requirements related to hearing and vision screening, the rest was history taking.
PAST MEDICAL HISTORY: Benign.
PAST SURGICAL HISTORY: Negative.
SOCIAL HISTORY: Patient is a non-smoker with no use of chronic use of alcohol documented.
ALLERGIES: No known drug allergies.
PHYSICAL EXAMINATION: Temperature 105.4 degrees, pulse 56, respirations 18, blood pressure 138/64, weight 74 kg, height 72, Oxygen saturation 97%. In general patient is a pleasant unconscious male Nobody in no acute distress. HEENT exam is unremarkable. Neck is supple without lymphadenopathy. Lungs are clear to auscultation bilaterally. Cardiac exam reveals a regular rate and rhythm. Normal S1-S2. There is a 3/6 holosystolic murmur heard best at the apex. It radiates throughout the precordium. Abdomen is soft, nondistended with normoactive bowel sounds. CNS has no focal deficit.
ASSESSMENT: Physical exam, cleared for Project Twilight. Regular instruments are proving to be useless and unable to withstand the internal temperature. Conversely, an attempt to set the patient on fire proved fruitless. The surrounding area will catch fire, but he himself will not. Incomplete exam due to interruption.
