I tried to re-create this icon,so the colors may not be exact.
First,resize and crop your image.This is your base.My base is from The Chronicles Of Narnia:LWW
Duplicate the base twice and set them both to 'Soft Light'.
Create a new layer.Fill it with #FFC6F0.Set it to 'Soft Light' at %70 opacity.
Create another layer.Fill it with #ABFFF5 and set it to 'Color Burn' at %55 opacity.
Then duplicate the base.Bring it to top.Desaturate it.Take the Delete Tool and delete half of it.(If I couldn't explain.Sorry,My english is not very good!)
Then duplicate 'Color Burn' layer and bring it to top.Set the opacity to %21.
AND VOILA!!YOU'RE DONE!!
