Dean gets sick enough he needs to be on some heavy-duty antibiotics or some sort of medication that he can’t drink with due to the side-effects. Maybe he sneaks a drink (or five) and it doesn’t end well or maybe he refrains and it... doesn’t end well.

I'd like it to be set in the later seasons to post-series, and you could also add something with mild to severe alcohol withdrawal - the latter of which can cause fever ALSO, fyi.
