Summary: Well, certainly true madness must be preferable to perceived madness. It is only a question of how he is to distinguish between the two, given all that has happened.
In this chapter: John takes time out of his busy schedule (consisting primarily of telling himself he's gone completely out of his mind) to catch up with the plot. Meanwhile Watson studiously attempts to avoid it. One is more successful than the other.
