-John involved in an abusive relationship (read the one about sexually abusive "tom") either in the past or presently. Sherlock discovering this.

-John having an abusive childhood. Again, Sherlock discovering this.

-John being diagnosed, fighting and/or recovering from cancer.

-John recovering from rape or assault(read blind_author).

-John seriously ill or wounded.

-John's parents disgusted upon learning he is in a relationship with Sherlock. Sherlock "dealing with" his parents.

-John dying.

-John kidnapped.
