"God, Sammy, please stop crying." Dean kneels down next to the bed, brushes Sam's sweat-soaked hair out of his eyes. The fever seems worse than it was earlier, even though he's been giving Sam children's Tylenol all day.

"But it hurts," Sam wails, his tear-filled eyes beseeching Dean to do something, anything to make it stop.

Dean stands up, takes a deep breath. "Okay," he whispers to himself. "I'll take you to the doctor, okay? They'll make it better."

Sam nods, sniffles, and gingerly sits up, clutching one hand to his aching belly. Dean grabs the keys from the dining room table and picks Sam up. It's a long walk to the hospital, but they don't have any choice. At least the hospital isn't all the way across town. The streets are deserted this time of night, that'll make it a little easier.

Dean locks the door behind him and starts carrying Sam down the street. Sam's tears soak through his shirt as Sam cries silently, miserably on his shoulder. "It's okay, Sammy," he says quietly, although it really isn't. "It'll be okay."

Half an hour later Dean carries Sam through the sliding glass doors of the emergency room. The woman behind the counter frowns. "Where are your parents?"

Dean stops in front of the counter. "Can you help my brother?"

An older nurse comes out from behind the glass doors. "What's wrong with your brother, honey?" she asks, kneeling down in front of them.

"He's got a real bad stomachache. And a fever. And he's been throwing up," explains Dean. She reaches out to feel Sam's forehead but Sam buries his face in Dean's neck and whimpers. "It's okay, Sammy," he whispers. "She can help you."

The nurse stands up. "Come with me, boys." She puts a hand on Dean's shoulder and leads them to an exam room. When she pulls Sam out of Dean's arms to sit him on the exam bed, Sam screams.

Dean clambers up on the bed and slings an arm around Sam's shoulders. "Shhh, it's okay, you're okay."

The nurse takes Sam's temperature and blood pressure and asks him a bunch of questions that Dean answers while Sam hides his face in Dean's shirt. She writes on a clipboard and promises that the doctor will be in to see Sam soon.

The doctor comes in about five minutes later with a little blonde lady who isn't wearing a white coat or nurse clothes. Dean's stomach clenches. She has to be a social worker. They're in trouble.

The doctor frowns at Dean. "Get down, please." Dean jumps off the table but grabs Sam's hand to calm him down.

The social worker smiles. "What's your name?"

"Dean."

"Hi, Dean, I'm Karen. Can you come out in the hall with me?"

"No." Dean glares at her. "I'm staying with my brother."

"Dean, the doctor needs to examine your brother."

"I won't get in his way." Dean squeezes Sam's hand when Sam's crying increases in volume. "I'm not going anywhere, Sammy."

The doctor reads the clipboard and moves to the bed. "Sammy, can you lie back for me?" Sam sniffles and does as he's told. The doctor asks him a few questions and then presses down on his belly, which makes Sam howl and kick the doctor's arm. The doctor doesn't even get mad, just picks up the clipboard and starts writing.

Dean scrambles up on the table again and puts his arms around Sam. "Shhh, it's okay, he's done now," Dean murmurs.

The doctor clears his throat and explains that Sam has appendicitis and needs an operation or else he could get a lot sicker, but they can't do an operation without a parent's permission.

"But it's an emergency," says Dean. "Can't you make an exception?"

Karen steps forward. "Where are your parents, Dean?"

"I need a phone," says Dean.

"There's a phone in my office," replies Karen. "Come to my office and I'll let you call whoever you need to."

"Not without Sammy." Dean holds her gaze, daring her to deny him.

The doctor's face softens. "I'll get a wheelchair."
