I am writing to offer you a position as my research assistant. Information regarding your appointment is indicated below:

Position: Research Assistant
Salary: 523 Galleons, 12 Sickles per year

This appointment confers medical coverage under the Subordinates and Peons Act of 1212 issued by the Department of Health and Safety at the Ministry of Magic. You will also have the opportunity to enroll in the Research Employee's Catastrophe Insurance Program to compensate you in the event of loss of limbs. You are officially granted five days of vacation. Travel will be expected and compensated.

The Werewolf Registry and Werewolf Reform and Control Act of 1936 requires employers to hire only beasts who are eligible to work in the United Kingdom. Please complete Part 1 of the enclosed form W-9 and bring the form and original documents as specified in Part 2 (List A or B or C) with you when you report for duty. The WQU-85, Questionnaire for Non-Sensitive Ministry-Funded Positions, is used to initiate a Werewolf Registry Check with Inquiry background investigation. Accurate completion of this form is a condition of employment. Please bring the completed WQU-85 when you report for duty. The questions must be completely and properly answered according to instructions. You must initial and date all changes including corrections, scratch outs, or if you've used Erasing Charms. The Department of Non-Human Employees will not accept or process any incomplete forms.
