To answer all your points at once, the 25-in-1 fic is supposed to be a one continuous work blending all the prompts; you don't need to tally up the prompts and devote a section to each of them, or something, and there's no words quota on each separate prompt. For example, if you have 'arranged marriage,' 'learning to be loved', 'loss of voice' and 'dungeons', your story can absolutely be about a pair of people in arranged marriage teaching each other to be loved via gestures and notes while imprisoned in a dungeon :P
It's generally expected to have all the prompts be at least somehow present in a story, but flashbacks and such are okay! Like, if there's a 'fever' prompt, somebody should have a fever or they had a fever and it was described, but it won't do to just mention a possibility of somebody developing a fever in a conversation and making on.
