I can't imagine they're anything less than a hassle to deal with when they're a part of you, even. But then, I wouldn't know.

The use of a bra is highly advantageous, however it becomes more of a problem the larger they are.

Hm. It should be fairly easy, to a trained surgeon anyway. We don't have very many of those where I'm from, though; most healing is done by Seventh Fonists, but occasionally additional intervention is necessary.
