For 750ml boiling water:
5-7 leaves lemon balm (melissa)
5-7 leaves verbena (louisa)
3-7 leaves sage
half a teaspoon, flat, of lavender
4-5 small rosemary needles
If you look at the melissa, louisa and sage and think that this is a cousin of Clarity, you're right. It's a fairly distant cousin, though: sage here is mandatory and not optional, spearmint should definitely not be added.
Modified off of Clarity means that it's a work-time infusion. The sage and the rosemary are there to balance the lavender, and the addition of lavender to the lemon balm and louisa is supposed to rerout nervous or anxious energy. The Clarity infusion takes available energy and directs it towards a lucid, easily-focused state; if you're starting from a reasonable state it's better than coffee, but if you're starting from a thin-skinned state it's not going to do much good.
