5) List your bad habits and/or addictions and what you have tried to rid yourself of them.
I have a problem with watching too many Television shows. They are so stupid to me and yet I continue to watch them week after week, and do that instead of studying and doing work.
Also, I have an eating problem, I eat too much junk, and I don't work out.
I have tried to limit time with television, and portion myself with food, and work out for at least half an hour a day, but I always just get lazy.
I am really determined to make all of this happen, but I don't know if I am strong enough, or believe enough in myself to do any of it.
