Have you died in Facility? If yes, how many times?: Yes. Twice.
May Death know about your Facility deaths, if so may he mention them to you?: Sure. The first was during canon swap week. He had been walking with Alice Baroque who was switched into the Bleach canon. She was a plus at the time, but turned hollow and devoured Tamaki. This was a particularly traumatizing death for him since not only was it the first time he had that sort of violence against him, but also since he was able to see his blood and organs leaving him through her transparent tubes. When he woke, it also left scars on him which damaged his own personal status of flawless beauty. He's still ashamed by the 4 circular scars on his body and won't show his bare torso, when before he would have loved to.
The second time was while he was infected by a clown. In order to stop the clown from attacking Xion, there wasn't any other choice but to have Xion kill Tamaki. It was done with blunt force trauma to the back of the head and neck with her key blade. This death wasn't as traumatizing for him, since it actually stopped him from hurting someone he cares about. He was more upset about what he had done while alive than his actual dying.
Bonus: Death may also know about his mother Anne-Sophie. She has lupus and was sick through most of Tamaki's childhood. Now, my Tamaki is from the anime canon which states that she works as a house keeper for the Tonair family. They didn't say what her state of health was, so that's up to you. If you'd like to pull from the manga canon instead, she lives a mostly active life as a part time piano teacher, undergoing clinical trials for the Ootari group in search for the cure to her illness.
If Death wishes to reference her, or speak about her as if he knows her, This is a good place to start. It's a flash back that is true regardless of which canon. In later chapters, Kyouya speaks to Anne-Sophie in person, if you want to pull from the manga. In the manga she still feels a great deal of shame for trading Tamaki for the guarantee of healthcare.
