Do you want your icon to look like one I've made before? If so, specify which one:

Rules:
1. Only the first 15 people will be able to request icons. If I get that many people requesting, that is. :p
2. High quality images only! I don't mind screencaps, but they have to be a reasonable size.
3. Please keep the text minimal. Too much text looks tacky, in my opinion.
4. You can request up to three icons.
5. Please fill out the form for each icon you want.
I'll post the icons in a new entry in about a week or two, depending on how long it takes me to get 15 requests. :p
