Elizabeth has HAD it. She is three whole years old now--she's a big girl!--and it is high time she has a puppy of her very own. She doesn't have much of a plan; she doesn't need one, because her daddies always give her what she wants. So she walks right up to Dad and lays it on him.
"Papa! I love you!" She flings her tiny arms around her seated father's distended belly. She closes her eyes and holds on tight, waiting for him to return the hug.
"I love you, too, princess!" he says as he squeezes her back fiercely and blowing raspberries on her cheek. Dad is a good hugger, even though he tummy is starting to really get in the way.
"Papaaaa..." Elizabeth stretches out her vowels as she looks up at her father with his own hazel eyes. "Can I have a puppy?"
Papa's eyes draw wide and begin to dance with amusement. A huge grin breaks out over his face and he begins to reply before he is interrupted.
"Angel," Daddy says as he walks into the den. "I thought we went over this before." He leans down to kiss his husband hello, before sitting on the arm of the counch and addressing her again. "We're getting you a new baby brother or sister. That's even better than a puppy!"
Elizabeth starts to pout. "But Papa is not done cooking the baby in his tummy yet! A puppy we could have now." Her logic is solid; why doesn't Daddy understand?
Papa just laughs and puts his hand on Daddy's knee. "She has been really good. And you said you would think about it." Daddy sighs. "Besides... you know a puppy is what I've always wanted." Papa grins and makes his eyelashes go all flutter-y. Daddy's face gets really soft at that. Daddy always has a hard time saying no to Papa's eyelashes.
Daddy considers his two favorite people in the world, then rolls his eyes. "Sweetie, why don't we just put your hair in dog-ears? Then you can be your own puppy."
"Honey, we both know I can't rock that look," Papa replies. Daddy laughs, but he gets up off the couch anyway and heads for the kitchen.
"Alright, I'll think about it after the baby comes... and we Scotch Guard all the furniture." Daddy disappears into the kitchen and Elizabeth hears pots clanking.
Papa just winks at her and tucks her between his arm and his side. She snuggles close and puts her hand on his belly, feeling for any movement from the baby. She guesses she can wait for an answer. Besides, once the baby is born, she's totally going to want a puppy, too. Between Papa, the baby, and her, Daddy will have to say yes. Now, it is just a matter of time.

Also, to answer your question: yes you do need to include both the theme and (at least) one prompt. However the theme is more of a general direction for your fic to go in whereas the prompt is much more specific and something you need to address directly in your fic (whether it's including a quote or following a specific story line, etc.)

This was a very important decision. Elizabeth was responsible for a whole new member of the family. Papa had done his job... well, kinda. He was supposed to have a girl, but instead he had a silly boy with a little, pointy head. Daddy and Papa promised her that he'll be just as good as a little sister--"you're a big sister either way, honey!"--but she wasn't so sure. She pouted so hard that they were practically begging to adopt a puppy for her, so she guessed everything worked out.
She walked through the yard looking at all the pretty puppies. There were a lot of good choices, but she wasn't sure which one was the right one for her family. Papa held her hand and led her around, while Daddy followed behind carrying baby BJ. Elizabeth was starting to worry that her new brother or sister puppy wasn't there, until a fluffy brown puff zoomed from the building and into the yard.
The puppy ran straight to Elizabeth, jumped on her, jostled her hand free from her papa's, and knocked her to the ground. Papa reached down with concern in his eyes to pull Elizabeth back up, but Liz was too busy laughing. The puppy licked her all over her face and neck and got paw prints on her dress. She petted him and giggled, "He has Papa's hair!"
"Poodles usually do," said Daddy, smiling. BJ gurgled. He thought it was funny, too!
"So this is the one you want, sweetheart?" Papa asked. "A chocolate poodle? I hear they are very smart. And they don't really shed."
"Really?" Daddy's eyes got bright. "Sounds good to me."
"I wanna name him BJ!" Elizabeth stuck her little fingers into the puppy's fur and pulled him close.
"But honey... that's your little brother's name."
"I know, Daddy!" They could share names and clothes.
"Lizzie, I really think he should have a different name, so we won't get them confused."
Daddy made a good point, but now she didn't know what name to give him. She scrunched up her nose in thought.
Papa could tell she was confused. "You could name him after something else you love. That's what some people do."
Oh! Well, that's easy. "Macawoni!" she cried.
Daddy and Papa looked at each other, then looked back at her and the puppy. "Macaroni?"
"I love macawoni!" Elizabeth and Macaroni rolled around in the grass of the yard, him yipping and her laughing joyously.
"Well, I guess we can call him Mack for short," said Daddy.
"I like it," replied Papa. "It's butch."
Elizabeth felt so proud. She had picked out the best little puppy-brother and the perfect name. Being a big sister was gonna be awesome.

Elizabeth is great. Lol, shame on Blaine for not popping out a girl for her. Next time! She'll convince them to try again for a little sister :P

Liz will def campaign for a little sister. Once the puppy grows up and her writing skills improve, she's gonna make doggie sandwich board signs. She'll also hand out flyers.
