These have been rec'd all over the place, but I'm going to add my two cents because these are all just wonderful and the authors deserve the credit.
I'm a sucker for hurt!/sick!boys. Much (if not all) of what you find here will fall under those categories.

Play Dead by
Mandy's Bitch is a very clever early casefile in which Sam becomes inexplicably ill after a hunt. It is longish, and the way Dean cares for his sick brother is touching. Gen.
That I Could Travel Light by
kres is Sam with amnesia and has a great twist at the end. Post Born Under a Bad Sign. Contains wincest.
Fire in the Hole, Red and Verbal by
Big Pink. All of her fics are great, but these three are my favorites. Her fics are available in pdf format

Fire in the Hole is a novel-length fic in which Dean is seriously injured on a hunt after Sam leaves for Stanford. Great examination of the relationship between Dean and John. Gen.
Dean is hunted by the Big Bad Wolf in Red. Actually quite terrifying. Gen
Verbal - Dean is cursed and can't stop talking. Gen. Humorous.
Open Your Eyes by
Dodger Winslow is pre-series hurt!dean. Another wonderful look the father-son dynamic between John and Dean after Sam is no longer in the picture. Gen.
Tired Most of the Time, Exhausted the Rest and its prequel
Running Around and Falling Off Things by
Kellifer_fic are humorous and slightly angsty list of injuries that Sam and Dean have sustained over the years. Gen. The rest of her fic is also very good and worth checking out.
A Good Man is Hard to Find by
kimonkey7 is a wonderful, heart-wrenching story in which Dean battles his own demons and an addiction while he and Sam work a case. Gen.
In Reverse by
sodakey is a long post Faith story. While Dean is recovering from his near-death experience, the brothers run into some of Sam's old friends from Stanford in Wyoming and end up facing a ghost that they may have faced ten years before. Gen.
