The royal muyst be mentioned by name or title: "King Tut" or "The Sultan or Jalalalbad" are both fine, but 'England's Rose' isn't.
The woman doesn't have to exist, no - however, "Hey Jude" wouldn't count, given it is commonly know to have been addressed to Julian Lennon :-)
