I agree on the thoughts of Sam growing up. There's been a streak in Sam of the rebellious teenager since the pilot and I think we finally saw that tempered in 5x02. Sam in 5x03 was so much more mature than he was in S1, or even S4 - it was beautiful to watch.
I very much believe that Alistair was messing with Dean in that scene. After all, Alistair was pretty keen on the concept of taking Dean back to Hell and reforming him into the image of his pupil once more. I've no doubt that he pulled psychological punches in Hell just as readily as he handled physical knives. It would have done him no good to tell Dean the truth back in Hell - before Dean broke, it would have fortified him against the torture, afterward, he was intent on "carving" Dean into a new animal and shattering him wouldn't have aided Alistair in that goal. But once Dean had managed to get away and dared to strike out against Alistair? Oh, absolutely, that called for some retribution and Alistair KNOWS Dean inside and out. He knew exactly what would have caused the most pain and done the most damage.
I like that you point out that said truth would have caused more pain than any physical torment as well. That fits in quite readily with Alistair's general distaste for being on Earth and his statement that you can't cut deep enough into flesh. You can torture the body without actually torturing the soul within (as Alistair proved himself quite aptly) and his very first sally into the fight was a slam against Dean's core, not his flesh.
Castiel IS pushing blame and you're right, it's huge that he was able to move as far as he did. I read a really good meta that pointed out that Castiel is a being that's thousands of years old and has been, it's implied, unchanged throughout that time. Now he's evolving and quickly; of course it will be painful and troublesome to watch and of course he's going to make some bad judgment calls in the process. He's got to be terribly confused which is what makes his determination that much more inspiring and impressive. *pets him*
