Question 4 Two players have just kicked off some of the most awesome roleplaying you've seen...?
(i) Not assume they are bored; they might have decided you do not get to suddenly turn up just because it is cool. (ii) Finish my coffee and place my mug outside the GM screen to indicate that bored people could put the kettle on. (iii) Start a new mental thread to confirm that opportunities are equal for all players
Question 5 You are running a well known game about bloodsuckers....?
It depends on the level of the game. As part of character generation I define acceptable levels for these things, with a reason. If a player has a good reason that they can give for exceeding these, and they ask first, then they get a fair chance to convince me. To extend the answer into the more general concept behind it, if all my players turned up with a particular stat at a very high level I would use it to slant my plot so (i) they get to Odin's Beard at that thing and (ii) the challenges are in other areas
Question 6 One of your players is having a really hard time....?
(i) Be the change they need to feel. If the players are happy with the way it is going them cut them some slack and get back to doing what you want when they have worked some of it off. (ii) Slant the snack supply so that (a) there is more of their favourite things and (b) more than just quick burst sugars and pop. (iii) If they are taking the game away from the source of enjoyment, then raise it with them privately
