Traditional, demons are the same sorts of creatures as angels. Demons are just angels that swapped sides. In the Supernatural universe, it seems like the higher grade demons were angels previously. Uriel's words underlined this. Traditionally (and I don't think the series has said anything to contradict this) Lilith was originally sort of human and sort of not. She was created as Adam's wife, but as his equal, unlike Eve who was created subsidiary to Adam. Lilith rebelled against her proposed inferior position and then took up with Satan (Lucifer, whatever) and became the mother of all demons (or vampires). Either way, she has a particular dislike of children, which does fit somewhat with the show's representation of her having a fondness for possessing little girls.
I was surprised that Castiel wasn't noticeably freaked out by Sam's latest accomplishment. I'd say his lack of reaction could be interpreted as an indication that this does not mean that Sam can kill angels.
Alistair is the highest grade demon we've seen Sam go after, and we don't know if he was Fallen. Still, he was a white-eyed demon, which bodes well for Sam's ability to take on Lilith.
