Probably somewhere inbetween? Like in the books, Face figures out that one of his squadmates is Not What She Seems because he notices over time that there's a discrepancy her conscious and her unconscious/relaxed body language that doesn't really make sense (and then follows it up through other channels because he only acts like an idiot), but at the same time he picked up on the person-suitedness (his exact words were “Something like being stroked by a poisonous reptile wearing a human suit, only not quite so comforting.”) of someone he met when he was still young enough that being sat in their lap was an appropriate thing.
So for Hannibal it'd likely be a combination of picking up on Hannibal calculating everything (even the stuff normal people don't need to think about) and semi/sub-consciously realising he moves like a predator (because mmm...Mads Mikkelsen's physicality), but the overall effect would be less "Doctor Lecter is clearly eating people, let me audition for the Chilton/Graham production 'I Told You So'", and more "Doctor Lecter freaks me out, I'mma avoid him, you should too".
