My local theater double-checks tickets on R-rated movies at the theatre door.

Darned if I know what they employ those guys for while the movie is actually running, but hey.

At Famous Players, at any rate, they make sure that your ticket corresponds to the theatre you're entering. And they're very careful about age stuff. (Though my Grade 9 class went to see an NC-17 movie - Shakespeare in Love.)

I used to work at a movie theater, and while I can't promise this is what always happens, but here is what's supposed to happen. If you try to buy a ticket for a movie you can't see, then buy a ticket for one you can, the ticket they give you will be marked (Our theater would print stars around the title). This mark tells the ticket collector to watch which theater the customer goes into.
