Step 1. the base
Select a picture. Crop it 100x100. Copy Base and put it on Screen opacity 40%

Step 2. Layers

Pick this texture layer from colorfilter  which I've cropped and put it on hard light. Add a layer mask and start erasing the parts that cover your original base on this texture layer with a soft brush, depending on the opacity of the brush.

Take this texture layer, again from colorfilter , and put it on darken. Add a layer mask and start erasing the parts that cover the face.

Add another texture, also from colorfilter , put it on hard light and add a layer mask. Erase the light that cover the face of your base.

Step 3. Finishing parts

Duplicate base and drag it on top of all your layers. Put it on Soft Light.

Take this dark-red gradient (creator: unknown, if this is yours please pm me to tell me that so that I can properly credit you) and drag it on top of all the other layers. Again, Soft Light.

Step 4. Adding extras

Use the eyedropper tool, pick any color from your now-almost-finished icon and then write any text with this color in the foreground. Use a music note brush as a finishing touch

The end.
