Menstrual symptoms on T

*Period related*

Background: A week on T (2% compounded cream), 3 years on a progestin based BC (Depo provera, Implanon - currently on both as I switched to Implanon while the DP was still in my system, will be gone by October) for treatment of endometriosis.
Spotting comes and goes, right now I'm in a period of spotting for the past three weeks.
Pre-T blood work was OK, but low estrogen levels and high iron levels.
Early 20s and underweight.

So just a week on T, I've suddenly got pains in my lower back and pelvis as well as bloating, similar to my period symptoms before I had progestin treatment. I haven't had these symptoms since taking progestin. The spotting is ongoing and I'm unsure as to whether it has increased in flow, but doesn't look like it was at the same level as pre treatment (VERY heavy). No sexual activity that suggests pregnancy. Also since I haven't had a normal period in 3 years, don't know whether this is my period or just part of the spotting.

Is this an expected symptom or sign of a problem? I'm worried the testosterone is aromatising to estrogen, which aggravates the endometriosis. It's a low dose though.
Not sure whether I can get a hold of my endo. I have never seen a doctor for ongoing treatment before so don't know whether one can just speak to the doctor on the phone rather than book an appointment?
