Tara walked into Buffy's life by accident, through Willow. The two of them had never been close friends, save the breakdown involving Spike.
It was so easy to blame Warren for everything because he had headed the Nerd Troika and it was his gun. He had fired the gun but the bullet that minced Tara's heart was meant for her. It was her, Buffy, that had underestimated Warren and laughed at his magic balls.
Shew had thought he wasn't a real threat. Maybe there was nothing she could have done but Buffy couldn't help but apologize silently every time she saw a picture of Tara.
