Not surprisingly, after the pain of A Hole in the World, this episode sees our heroes reeling in the aftermath. The aftermath of not only Fred's death, but the rising of Illyria in her place - in the most literal sense possible. Over the years in the Jossverse we've seen vampires and demons aplenty; a god; a beast blotting out the sun - but we've never seen an Old One before. And here she/it is.
Angel and Spike are returning from their great mission. Which failed. They aren't good with failure. Gunn, meanwhile, is working out just who precipitated Fred's death - Knox, and even more so himself - and what he might do about that. Wesley is also working that out, with bloody consequences.
And among them, a God King of the Primordium is returned, seeking her ancient army and her ancient purpose. Keyword here, though? Ancient.

ILLYRIA My name... (approaches him angrily) You would presume to speak my name... Because I have returned in the body of the human, you think you can speak to me. It's disgusting.
WESLEY Who is Winifred Burkle?
ILLYRIA I thought the humans would have long died out by now. Instead, you've grown bold.
WESLEY So you don't know who Fred is.
