Background

CADASIL stands for cerebral autosomal dominant arteriopathy with subcortical infrarcts and leukoencephalopathy. It is a disease of young adults and presents with migraines with or without an aura, mood disturbances, focal neurologic deficits, strokes, and dementia. Most patients will show symptoms by age 60 years. There are recurrent subcortical ischemic events causing permanent deficits in as many as 2/3 of patients. Unfortunately, there is no laboratory screening test except for a mild elevation of cerebrospinal fluid protein in 25% of cases. Recently, electron microscopic studies preformed upon a skin biopsy may yield the diagnostic changes.
