If he were to have a sex change, the surgeon would do extensive work on his face and neck. I found some interesting information on the subject on a website which had great previews of what someone would look like after surgery, and an explanation of all the various techniques. I think they could make Hugh look pretty, or at least not bad. I mean, look at Izzard's before and after! For one thing, he wouldn't have the heavy blue shadow from his beard, because electrolysis would get rid of that. His lips would be plumped up which would make his lips look feminine AND make the space between his nose and upper lip shorter, which would also enhance the feminine look. He'd be given a brow lift, and then his eyebrows would be thinned to a more feminine shape. His chin would be both brought forward and shortened; his scalp brought forward to hide the masculine slope of his forehead; some forehead recontouring to de-emphasize the brow ridge, and many other adjustments.

The changes are extensive in some cases (not how much their jawlines change and their foreheads) but one thing is clear: if you're a handsome man, you'll be a pretty woman, and vice versa. Except for Leonardo Dicaprio. Oddly enough, he looks worse as a woman, despite his rather feminie appearing face. Perhaps it's his nose. Kate Winslett and Johnny Depp look stunning as members of the opposite sex. There would be some serious facial surgery to achieve those results. HRT would take care of some of the differences.

Challenge:
Perhaps some artists/photo manipulators out there could play around with Hugh's drage picture using the techniques mentioned on this site to see just how much could reasonably be done to make him pretty? I might too. It would be interesting. Would be nice to find one in which he doesn't have such a goofy smile too.
