Last name, hell yes. It's not spelled "nielson," people, it's spelled "nielsen," as in nielsen ratings, leslie nielsen, and I'm Danish, not Swedish

Probably not, it's a surefire way to point out that you're saying something incredibly offensive. The last time someone started a sentence "No offense, but..." when speaking to me, it was with the deliberate intention of making me feel godawful. So I try to avoid it myself.
