Okay, I'm watching the first episode of season 3, and there's no one else in the room to tell how much I like this scene. House has a patient he thinks is faking her symptoms. You can tell he's disappointed because there's no mystery and she's just fine, so he takes it out on her by whipping out a huge needle he'd have to use on her if she wasn't faking. But when she doesn't stop and he notices distended veins in her neck and he has to use the needle to pull some blood from her chest cavity, it's disturbing and and a perfect characterization the gleeful grin on his face when he realizes he really does have a puzzle to solve, never mind this meaning the patient really does have something wrong with her.
It's just the facial expression, but it's absolutely perfect acting.
