Wow, this is one impressive bit of fic. I love the power struggle you've set up between these two, not to mention crazy-brilliant McCoy running his experiments behind the scenes. *g*

Oh he doesn't kill his patients (well, expect for his father, but those were different circumstances). He has some strange honor code where he won't kill someone in his care, but once someone isn't his patient anymore, they're fair game.
