From what you're saying, the way the blouse looks, and from my own experience, it would seem that your bust is too wide and/or your shoulders too broad.
Also, there's a good chance your arms are larger than you think they are. If your arms are flat at your sides, you are not flexing any of your muscles, and they will be the smallest width. If you stretch your arms out making a T-shape, you're flexing your triceps, which are smaller, but do increase the circumference of your arms a bit. Finally, if you lift your arms up over your head, you're flexing your biceps, which are the biggest muscles in your arm, and therefore increase the circumference of your arm by about 1-2 inches on someone who isn't a body builder but does use their arms hehe. I have this problem as I lift weights and my biceps make my arms about 2-3 inches more circumference flexed. I recommend fabric with a bit of stretch, or blouses with shirring around the arms, if you discover this is part of the problem.

It might be okay, I'm not sure, but it looks like it might be raised slightly at the shoulder seam. If the shoulder seam doesn't lay perfectly flat on me, it makes my shoulders look very broad and AWFUL. Poofy sleeves that poof very high up are bad too. I can do poofy sleeves if the poof starts lower, below where the shoulder is. Otherwise I look manly. >_<
