I react to the epinephrine that is included in novocaine. Total exhaustion for at least a day if the dentist has slipped up and given me novocaine instead of Xylocaine (spelling?) -which is novocaine without the epinephrine. I have gotten arguments from dentists because they sometimes have to give me an additional shot as the epinephrine keeps the novocaine in place a bit longer, but avoiding the next day adrenal exhaustion is well worth it. (My current dentists take my reaction seriously, which is why I switched to them).
