It's not that they ignore her, exactly. Jo wouldn't be here if she even suspected it might be like that, because pretty as they are, she has some self-respect, thank you very much. Quite a lot of self-respect even. But she did always know that it'd be as much about them as it is about her, probably more, and she decided she could deal with that.
Actually, there's something molten and hot in the pit of her stomach to watch them together, a secret only she shares. It should be a sick-hot feeling, but this isn't like that. This isn't about flaunting anything or going against society, this isn't about rebellion. It's just about love. She doesn't think that makes it right, exactly -- Sam and Dean are still brothers, after all, and nothing will ever change that. You can even see the brotherly rivalry in the biting kisses, in the way they compete. She has no objections to the way they compete, since it involves making her feel so damn good.
But this doesn't dirty their brotherhood, somehow. It's just love, need, co-dependence, and it's all kinds of unhealthy, but Jo can't say it feels wrong to her in any fundamental way. It makes them too happy -- there's a kind of bliss on Dean's face when Sam is deep inside him, possession and almost-wonder on Sam's when Dean sucks him off, even when Dean's just touching him, even when he's just watching Dean with her.
Maybe they do ignore her, a little. But she's sharing in their secret, seeing something no one is allowed to see, and she couldn't explain why, but it's almost an honour to be a part of it, an honour to have helped them get this far.
